CoPAC provides a team-based approach to assessing the health, mental health, physical, social, and spiritual concerns of people with chronic pain. In January 2019, CoPAC was created as part of the Indiana University Grand Challenge: Response to Addictions Crisis and through the collaborative efforts of IU IPE, IU health and social professionals schools, and IU Health/Methodist Family Medicine Center. Using an interprofessional education and collaborative practice model, patients meet with providers and health professions students to review their goals and create a comprehensive care plan to address their needs.
The CoPAC team includes behavioral health, chaplaincy, dentistry, nursing, medical assistants, medicine, occupational therapy, pharmacy, physical therapy, and social work. Overall, the interprofessional education and collaborative practice model involves all team members (i.e., providers and learners) valuing each other’s roles, sharing responsibilities of patient care, embracing collaborative communication strategies (e.g., huddles, check-backs, and handoffs), and coordinating referrals and care plans to improve patient and provider satisfaction.
